Prior to your first session, please print, review and complete all forms below, and bring them to that appointment. The initial session is one hour. Subsequent sessions are 45-60 minutes.
For primary patients that are minors (under age 18), please complete the Child/Adolescent Information Form and bring it to your initial session. Please be aware that the legal parent or guardian of the minor must sign the consent forms.
